DECLARATION OF CONSENT

Kao nositelj roditeljske odgovornosti ili odgovorna osoba koja ispunjava sve zakonske pretpostavke u smislu poslovne i druge sposobnosti za davanje predmetne privole, dobrovoljno, prisebno, informirano i nedvosmisleno izražavam svoju suglasnost Voditelju obrade osobnih podataka Udruge Muzika u koracima, Brzac 97, Krk za obradu osobnih podataka koji se odnose na mene i/ili moje dijete.

I give this Consent with regard to the following personal data:

1. ime i prezime djeteta ili mene, datum rođenja, adresa, OIB
2. fotografije, snimke, video i audio zapise djeteta nastale tijekom odvijanja radionica udruge Muzika u koracima
3. izdavanje potvrde o pohađanju programa radionice

for the purpose of organizing the Association’s activities.

I am informed that the subject’s consent is the legal basis for this data processing.
The subject hereby allows the Head of data processing to use personal data for the relevant purposes by:

• entering the aforementioned personal data in the data processing register kept by the Association
• being forwarded by the Association to data sub-processors (Internet providers, server maintenance, etc.), the Association’s websites and social media pages for the purpose of public announcements. Personal data will be announced publicly if such a need occurs, and will be permanently stored for the Association’s statistical and archival purposes, or until the Consent is withdrawn, in which case they will be deleted in accordance with the law.
• Sukladno Zakonu o provedbi Opće uredbe o zaštiti podataka („NN“. br. 42/18.) dajem privolu (suglasnost) radionicama da zbog obavljanja redovitih poslova u okviru zakonom utvrđene djelatnosti, može prikupiti, obrađivati i koristiti moje osobne podatke fotografije, audio i video zapise te ih objavljivati na mrežnoj stranici Udruge Muzika u koracima, tiskanim materijalima te društvenim mrežama i kanalima. Ova privola daje se u svrhu dokaza zakonitosti obrade osobnih podataka i važeća je do trenutka osobnog povlačenja.

I have been informed of my right to withdraw the given Consent at any moment, which does not impact the legality of the processing that was based on this Consent before its withdrawal, to ask the Head for the access to personal data, correction, data deletion, limitation of processing, with the right to lodge an objection to the processing, the right to data portability, the right to submit an objection to the competent authority (Croatian Personal Data Protection Agency) and to send inquiries to the official of the data protection Head to udruga.mook@gmail.com with the note “for the data protection official” in case I believe there have been any irregularities in the processing of personal data, and I can also use the same e-mail to withdraw the given Consent.

This Consent is given with the purpose of proving the legality of personal data processing and is valid until withdrawn, after which the respective data will be registered exclusively for the purpose of proving the given Consent.
